Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state RN license is required.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate all aspects of the patient's home care and interact with medical teams as needed.
• Perform admission visits to determine eligibility for home care services.
• Develop and follow the plan of care for each patient.
