Ohio Living Cape May is seeking an experienced Home Health RN Case Manager to plan, coordinate and deliver quality home health care in compliance with state and federal guidelines. Qualifications

• Current unencumbered license for the state of Ohio to practice as an RN.
• One year experience in acute care or medical-surgical nursing required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Responsabilités

• Care Management and Service Delivery - provides ongoing assessment and nursing care to patients in homes and health care facilities; develops and implements a comprehensive care plan.
• Quality and Compliance Management - audits and maintains patient records and participates in quality improvement projects.
• All other duties as assigned.
