Job summary

Otterbein Home Health is seeking a registered nurse to provide patient-centered home health services across the greater Lebanon, OH area. The role includes initial assessments, care planning, medication administration, and family education in a community-based setting.

Require Ohio RN license and CPR certification, with a minimum of 1 year nursing experience; valid driver’s license and auto insurance are required for travel between patient homes.

Qualifications

• Bachelor’s degree in Nursing preferred; graduate of approved nursing program.
• Valid Ohio RN license and CPR certification.
• Minimum 1 year clinical nursing experience; home health or hospice preferred.
• Must have a valid driver's license and automobile insurance; clean driving record.

Responsibilities

• Complete an initial assessment of patient and family to determine home care needs; provide a full physical assessment.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family; provide health care instructions per assessment and care plan.
• Identify discharge needs and coordinate with the care team prior to discharge.
