Job summary

Otterbein SeniorLife seeks a registered nurse for home health visits across Lebanon, Middletown, Springboro, Dayton and parts of Cincinnati. You will assess patients, develop and adjust care plans, and coordinate with physicians and families to deliver compassionate, person-centered nursing care in patients' homes.

This role requires an Ohio RN license, CPR, 1+ year of clinical nursing experience, and a valid driver's license with insurance.

Qualifications

• Bachelor’s degree in Nursing preferred; graduate of an approved nursing program.
• Valid Ohio RN license and CPR certification.
• Minimum 1 year of clinical nursing experience; home health or hospice experience preferred.
• Must be licensed driver with a safe driving record and automobile insurance.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnoses and goals.
• Delivers preventive and rehabilitative nursing actions with family involvement.
• Administers medications and treatments as prescribed; provides counseling.
• Instructs patients and families on care plans and health maintenance.
• Identifies discharge planning needs and documents updates.
• Prepares clinical notes and communicates with primary physician every 60 days.
• Coordinates with community health professionals to execute the care plan.
• Participates in on-call duties and ensures equipment and services are arranged.
