Job summary

Ohio Living is hiring an RN Case Manager for Home Health to coordinate and deliver high-quality in-home nursing care. You will assess patient needs, create care plans, and collaborate with the care team to ensure adherence to standards and regulations.

The role requires an Ohio RN license, at least one year of home health experience, and proficiency with Windows and MS Office; EMR experience is a plus. A valid driver's license is also required.

Qualifications

• Current unencumbered license to practice as RN in Ohio.
• At least 1 year of home health experience required.
• Experience in Medicare-certified home health agencies preferred.
• Proficiency with Windows, Word, Excel, PowerPoint and internet required.
• EMR experience preferred; computer literacy essential.

Responsibilities

• Provide ongoing nursing care in patients' homes.
• Develop and implement comprehensive care plans.
• Educate patients and caregivers on conditions and therapies.
• Communicate changes in condition to the care team.
• Oversee plan of care implementation and evaluate effectiveness.
• Maintain accurate patient records and documentation per regulations.
• Participate in quality improvement activities.
