Job summary

Otterbein SeniorLife seeks a Registered Nurse (RN) Case Manager for Home Health serving Lebanon, OH and surrounding areas. The role focuses on comprehensive patient assessments, care planning, and coordination with families and physicians to deliver high-quality, person-centered home health services.

You will monitor patient progress, adjust plans as needed, administer treatments, educate families, and participate in on-call coverage. A valid Ohio RN license and CPR certification are required.

Qualifications

• Bachelor’s degree in Nursing preferred.
• Valid Ohio RN license with CPR certification.
• Minimum of 1-year clinical nursing experience; home health or hospice preferred.
• Must be a licensed driver with reliable vehicle.

Responsibilities

• Performs an initial patient and family assessment to determine home health needs.
• Initiates and revises the plan of care as patient needs change.
• Administers medications and treatments as prescribed; counsels patient and family.
• Coordinates discharge planning and communicates with physicians and caregivers.
• Participates in on-call duties as defined by policy.
