Job summary

Otterbein SeniorLife in Ohio is seeking an RN Case Manager for Home Health. This role travels locally across Lebanon, Middletown, Springboro, Dayton and parts of Cincinnati, delivering patient-centered care.

You will assess patients, plan and coordinate care with physicians, mentor home health aides, and participate in on-call duties. Requires RN license and at least 1 year nursing experience; driving is essential.

Qualifications

• Bachelor's degree in Nursing preferred; Graduate of approved nursing program.
• Valid Ohio RN license and CPR certification.
• Minimum of 1 year clinical nursing experience; Home Health or Hospice preferred.
• Must have a valid driver's license with automobile insurance and a clean driving record.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Initiates plan of care and revisions as patient status changes.
• Uses health assessment data to determine nursing diagnoses.
• Establishes goals based on nursing diagnosis and plan of care.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing needs and provides health instructions.
• Coordinates discharge planning and communicates with physicians.
