Qualifications

• Detail-oriented with ability to prioritize and follow directions.

Responsibilities

• Oversees patient care from start to end of episode.
• Updates doctor orders in the chart.
• Notifies doctor of significant changes in condition.
• Manages multi-disciplinary care.
• Provides skilled nursing care in patient’s home as ordered by physician.
• Administers medications as prescribed.
• Documents care and maintains regulatory compliance.
• Participates in on-call requirements and case conferences.
