Job summary

The Pennant Group, Inc. is seeking a registered nurse to plan, coordinate and direct home health services in Durango, Colorado. You will assess patient needs, develop individualized plans of care and educate patients and families in their homes and communities.

You’ll work closely with physicians, caregivers and the interdisciplinary team, supervise home health aides and LPNs/LVNs, and participate in on-call duties. Must hold an active nursing license in Colorado and CPR certification.

Qualifications

• Must have graduated from an approved school of nursing and be licensed in the state where practicing.
• Minimum of two years of experience, at least one in public health or home care nursing.
• Maintains a current CPR certification.
• Must be a licensed driver with a reliable vehicle and valid insurance.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Develops and initiates the plan of care and assesses the interdisciplinary care plan with patient and caregiver.
• Initiates nursing procedures and provides physician-ordered services.
• Provides patient, caregiver, and family counseling and education.
• Communicates with physician and health care practitioners about the home health plan.
• Identifies discharge planning needs and implements prior to discharge.
