Job summary

The Pennant Group, through its Symbii Home Health division, is seeking an RN Case Manager to coordinate and deliver skilled nursing care in patients’ homes. You will manage a caseload and develop individualized plans of care.

You will perform SOC/ROC/Recertification, coordinate with physicians, therapists, social workers and home health aides, ensure timely visits and accurate documentation, and monitor patient outcomes while adhering to Medicare/Medicaid requirements and HIPAA.

Qualifications

• Current RN license in the applicable state.
• Graduate of an accredited school of nursing.
• Minimum of one year of nursing experience.
• Valid driver's license and reliable transportation.
• Ability to travel throughout the assigned service area.
• Strong assessment, organizational, and communication skills.
• Basic computer and EMR proficiency.

Responsibilities

• Perform comprehensive patient assessments and develop care plans.
• Coordinate care among physicians, therapists, social workers and aides.
• Complete SOC/ROC/Recertification and related assessments.
• Maintain accurate documentation and ensure regulatory compliance.
• Educate patients and families on disease processes and self-care.
