Job summary

Qualifications

• Must be graduate of an approved school of nursing and licensed in the state.
• Minimum of two years of experience, at least one in public health or home care nursing.
• Maintains a current CPR certification.
• Must have a valid driver’s license with insured vehicle.
• Self-directed and able to work with minimal supervision.
• Demonstrates strong observational, problem solving and communication skills.
• Ability to organize and prioritize workload.
• Supervising home health aides experience not required.
• Able to lift, position and transfer patients; prolonged walking/standing.
• Visual acuity and hearing to perform required nursing skills.

Responsibilities

• Develops and initiates the plan of care and assesses the patient with the caregiver and representative.
• Initiates nursing procedures per physician orders and plan of care.
• Provides patient, caregiver and family counseling and education.
• Prepares clinical notes and updates the physician every sixty days as needed.
• Communicates with the physician and other health professionals about the care plan.
• Coordinates with community health personnel to implement the care plan.
• Identifies discharge planning needs and prepares for discharge.
• Participates in quality assessment and performance improvement activities.
• On-call duties as defined by policy.
• Instructs and supervises home health aides; supervises LPNs/LVNs/CNAs as directed.
