Zusammenfassung

The Pennant Group is seeking a Registered Nurse (RN) to plan, organize and direct home health services in the Utah region. The RN will use community resources to design patient-centered care for individuals and families in their homes and communities.

Responsibilities include initial patient assessments, plan of care development, ongoing evaluations, medication administration, patient and family counseling, and coordination with the Clinical Supervisor.

Qualifikationen

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree, with one year of home health care experience preferred.

Aufgaben

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Initiates preventive and rehabilitative nursing procedures and administers medications.
• Counsels patient and family in meeting nursing needs and provides health care instructions.
