Job summary

The Pennant Group, Inc. is seeking a Registered Nurse to direct hospice nursing in home settings, building from community resources to plan care for patients and families in their homes and communities.

You will perform comprehensive assessments, develop care plans, administer treatments, provide education, and coordinate care with the interdisciplinary team, while maintaining documentation and on-call responsibilities.

Qualifications

• Graduate of an NLN-accredited nursing program.
• Minimum two years nursing experience; hospice/home health preferred.
• RN licensure in the state.
• Excellent observation, problem solving, and communication skills.
• Ability to organize and prioritize workload independently.
• Must be able to lift up to 50 lbs and assist with transfers.
• Valid driver's license and access to transportation.

Responsibilities

• Assess patient and family to determine hospice needs and plan care.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family input.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Educate patients and families on self-care and home care routines.
• Coordinate patient care as Case Manager within caseload.
• Maintain accurate clinical notes and report changes to physicians.
• Participate in on-call duties and ensure equipment and services are available.
• Contribute to quality improvement activities.
