Responsibilities

• Utilizes RN process as a framework to focus the activities of the healthcare team on optimal outcomes and resource utilization.
• Interacts with patients, professionals, and the community to ensure continuity of care and document plans of care.
• Conducts comprehensive needs assessments for identified patients and matches services to needs.
• Develops and maintains accurate case records for referred patients.
• Documents fully and accurately using appropriate databases and documentation systems.
• Maintains knowledge of reimbursement criteria (Medicaid, Medicare, Managed Care).
• Demonstrates leadership in nursing practice and adheres to standards and guidelines.
