Qualifications

• Valid driver's license and ability to travel (~20% of workday).

Responsibilities

• Deliver skilled, patient-centered home health care in coordination with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Complete thorough EMR documentation and ensure compliance.
• Travel to patient homes within the assigned territory.
