The position offers a generous bonus program with potential total bonuses up to $30,000 over three years, including a $10,000 first-year success bonus, a $5,000 sign-on bonus, and additional monthly bonuses that grow with tenure.

Qualifications

• Active Registered Nurse license in the state you work
• One year of RN experience delivering quality patient care
• Field-based home health role requires valid driver’s license and travel ~20% of the day

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Ensure accurate EMR documentation and follow-up on care plans
• Travel to patient homes within the assigned territory
• Work independently while staying integrated with the team
