Job summary

Enhabit Home Health & Hospice in San Angelo, TX is seeking a dedicated Registered Nurse for coordinating comprehensive home health services. You will ensure care is delivered per standards, review patient information at starts of care and recertifications, and liaise with physicians, patients, and caregivers to maintain seamless care.

The role requires an active RN license, CPR certification, driving in Texas, and prior home health experience; familiarity with Homecare Homebase is a plus.

Qualifications

• Graduate of an approved school of professional nursing.
• Active RN license per state requirements.
• Minimum 2 years nursing experience within last 36 months.
• Minimum 1 year home health experience.

Responsibilities

• Coordinate the overall home health services per standards and procedures.
• Review/approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referrals, patients, caregivers and staff.
• Schedule patients with field clinicians and maintain communication to ensure care coordination.
