Job summary

Frontier Home Health in the Fort Worth area seeks a Registered Nurse to plan, organize and direct home care services with a focus on community health education. The role builds from community resources to meet home and family needs, coordinating with patients and caregivers.

The nurse provides education, coordinates with physicians, and participates in on-call and quality improvement activities to ensure effective care delivery.

Qualifications

• Must have graduated from an approved school of nursing and be licensed in the state where practicing.
• Minimum of two years of experience, at least one in public health or home care nursing is preferred.
• Maintains a current CPR certification.
• Ability to drive and have access to transportation for agency related travel is essential.
• Must have and maintain a valid driver’s license and automobile insurance.
• Self-directed and able to work with minimal supervision.
• Excellent observation, problem solving, verbal and written communication skills.
• Shows ability to organize and prioritize workload independently.

Responsibilities

• Develops and initiates the plan of care and assesses the individualized plan of care in partnership with patient, caregiver(s) and representative.
• Prepares and communicates clinical notes and coordinates with physicians and health practitioners.
• Supervises home health aides and other nursing staff as directed.
• Participates in on-call duties and quality improvement activities.
• Ensures equipment and services are available to meet care needs.
