Graham Healthcare Group in Monessen, PA is seeking a Registered Nurse (RN) – Home Health to deliver skilled nursing care in patients’ homes. You will perform assessments, care planning, education, and maintain EMR documentation in a fast-paced home health setting.

Qualifications

• Active RN license in the state of work.
• At least 1 year of RN home health experience preferred.
• Current driver’s license and ability to travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in patients’ homes.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Maintain thorough EMR documentation and ensure compliance.
• Work independently with support from the interdisciplinary team.
• Travel to patient homes within the assigned territory.
