Job summary

Graham Healthcare Group is hiring a Registered Nurse (RN) for Home Health in Riverside, PA. This ongoing staff position involves delivering skilled nursing care to patients in their homes, with travel within the assigned territory and a 40-hour work week.

You will perform assessments, manage medications, and educate patients and families about disease management, aiming for high-quality outcomes and compliant EMR documentation.

Qualifications

• Active RN license in the state you work.
• One year of RN experience and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
