Responsibilities

• Deliver skilled, patient-centered care in a home health setting with interdisciplinary team support
• Conduct start-of-care assessments, including clinical evaluations and medication reconciliation
• Develop initial care plans aligned with physician orders
• Educate patients and families through admission, set care expectations and review diagnoses
• Coordinate admissions independently while ensuring timely EMR documentation
• Travel to patient homes within assigned territory
