Elara Caring in Springfield, MA is seeking a Home Health RN Case Manager to guide patients from assessment to discharge with skilled nursing visits and care planning. You’ll manage an assigned caseload, coordinate with physicians, and teach patients and caregivers how to manage medications and conditions at home.

We offer a guaranteed salary of $88,000–$97,000 plus a productivity bonus, mileage reimbursement, comprehensive benefits, tuition assistance, and paid orientation.

Qualifications

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to travel throughout Springfield.
