Job summary

Elara Caring is hiring a Home Health RN Case Manager for the Northampton-Easthampton territory. This full-time role offers a guaranteed salary of $87,000 to $97,000, plus a productivity bonus, mileage reimbursement, and a comprehensive benefits package.

You will conduct skilled nursing visits, develop and update care plans, manage an assigned caseload, and coordinate with physicians, while teaching patients and caregivers how to manage medications and conditions at home.

Qualifications

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to travel throughout assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.

Responsibilities

• Provide skilled nursing visits and care plans in patients\' homes.
• Manage an assigned caseload and coordinate with physicians and other clinicians.

What You’ll Do

You’ll guide patients through their home health care, from assessment to discharge. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

• Ability to work independently and travel throughout assigned territory
• Valid driver’s license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
