Job summary

Elara Caring is seeking a compassionate Home Health Registered Nurse in Saugus/Lynn/Reading to provide personalized nursing care in patients' homes. You will manage a caseload, perform assessments, and ensure plans of care are followed, while educating patients and caregivers on diagnoses, medications, and self-care strategies.

You will travel within the branch coverage area, collaborate with an interdisciplinary team, and maintain accurate clinical documentation in a HIPAA-compliant process.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted Registered Nurse license in MA.
• At least one year of nursing experience in a clinical care setting.
• Associate Degree in Nursing required; BSN preferred.
• Home health/home care experience preferred.
• Willingness and ability to travel within the assigned branch coverage area.

Responsibilities

• Provide individualized nursing care in patients' homes.
• Manage a dedicated caseload and coordinate care with the interdisciplinary team.
• Perform comprehensive nursing assessments and update plans of care.
• Administer medications and monitor treatment response per physician orders.
• Educate patients and caregivers on diagnoses, treatments, and self-care.
