Elara Caring is hiring a Home Health RN Case Manager in Springfield, MA. This full-time role offers a guaranteed salary in the high-80k to mid-90k range plus a productivity bonus.

You will conduct skilled nursing visits, develop and maintain care plans, and coordinate with physicians while teaching patients and caregivers to manage health conditions at home.

Qualifications

• Current, unrestricted Massachusetts RN license.

What You’ll Do

You’ll guide patients through their home health care, from assessment to discharge. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

What You’ll Bring

• Current, unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment, communication, and time management skills
• Ability to work independently and travel throughout Springfield
• Valid driver’s license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
