Job summary

Inspired Home Care is seeking a dedicated Home Health Nurse to join our expanding team on a PRN/Part Time basis. You will manage a personal caseload, visiting patients in their homes and delivering high-quality, compassionate care.

Responsibilities include assessing patient conditions, performing vital signs reviews, wound care, IV therapy, patient education, and coordinating with therapists and physicians to ensure a comprehensive care plan. Travel to multiple patients per day is required.

Qualifications

• State RN license required.
• 1 year of experience in in-home care.
• Excellent communication and time management skills.
• Compassionate and friendly demeanor.
• Ability to travel to multiple patients per day.
• Preferred knowledge of WellSky Kinnser.

• Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy
• Dress or re-dress open wounds and assess the progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Speech Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests
