Inspired Home Care in Illinois is hiring PRN and Part Time Home Health Nurses to join our expanding team. You will manage your own caseload and visit patients in their homes.

Responsibilities include assessing patients, recording vitals, wound care, IV therapy, Oasis assessments, educating families, and coordinating with therapists and physicians to support each care plan. Requirements: active RN license, 1 year in-home care, excellent communication and time management, and willingness to travel

Formación

• RN license required.
• 1 year of in-home care experience.
• Excellent communication and time management skills.
• Compassionate and friendly demeanor.
• Ability to travel to multiple patients per day.
• Preferred knowledge of WellSky Kinnser.

Responsabilidades

• Assess patients' condition during visits and chart observations.
• Perform vital signs, Oasis assessment, wound care, and IV therapy.
• Dress or re-dress wounds and monitor healing progress.
• Educate patients and families on home health care strategies.
• Coordinate with therapists, physicians, and other care team members.
• Make recommendations for devices or tools to improve quality of life.
• Listen to patients and address concerns or requests.

You will be responsible for managing your own caseload, so time management skills and the ability to work independently are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. If you enjoy working one-on-one with patients outside of a clinical setting,

Home Health Nurse Responsibilities:

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy
• Dress or re-dress open wounds and assess the progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Speech Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests

Qualifications:

• State RN license required
• 1 year of experience in in-home care
• Excellent communication and time management skills
• Compassionate and friendly demeanor
• Ability to travel to multiple patients per day
• Preferred knowledge of WellSky Kinnser
