Job summary

Inspired Home Care in Homewood, IL seeks a dedicated Home Health Nurse (PRN/part-time) to manage their own caseload and provide compassionate in-home care. You will perform patient assessments, vital signs, wound care, and IV therapy, while coordinating with a full care team to ensure optimal outcomes.

Ideal candidates hold an RN license, have at least 1 year of in-home care experience, and demonstrate excellent communication and time management.

Responsibilities

• Dress or re-dress open wounds and monitor healing progress.
• Educate patients and families on home health care strategies and procedures.
• Coordinate with therapists, physicians and other care team members.
• Recommend devices or tools to improve patient quality of life.
• Listen to patients and respond to concerns or requests.

Inspired Home Care is a 5-Star Agency ACHC accreditedd rapidly expanding and we are hiring PRN and Part Time effective immediately. You will be responsible for managing your own caseload, so time management skills and the ability to work independently are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. Home Health Nurse Responsibilities:

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy
• Dress or re-dress open wounds and assess the progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Speech Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests
