Job summary

Inspired Home Care seeks a dedicated Home Health Nurse to manage a patient caseload in a PRN/Part Time role. You will assess health status during visits, monitor vitals, perform wound care, and educate families on care plans.

Travel to multiple patients daily is required, with independent time management and compassionate patient interaction at the core. This opportunity emphasizes independent work outside traditional clinical settings, offering flexibility and patient-centered care in home

Qualifications

• State RN license required.
• 1 year of in-home care experience.
• Excellent communication and time management skills.

Responsibilities

• Assess patients' condition during every visit and chart observations.
• Perform evaluation tasks including vital signs, medication review, Oasis assessment, wound care and IV therapy.
• Dress or re-dress open wounds and assess healing progress.
• Educate patients and families on home health care strategies and procedures.
• Coordinate with Occupational, Physical and Speech Therapists and physicians involved in the care plan.
• Make recommendations for devices or tools that might improve quality of life.
• Listen to patient concerns and respond to requests.

You will be responsible for managing your own caseload, so time management skills and the ability to work independently are essential. Salary: $110 per visit

Home Health Nurse Responsibilities
