Job summary

Inspired Home Care, LLC is seeking a Home Care RN for PRN and Part Time roles in Illinois. You will manage your own caseload, visiting patients at home and providing intermittent skilled care with compassionate, attentive service.

Responsibilities include assessing patients, vitals, wound care, education for families, and coordinating with the care team. Must hold State RN license and be able to travel between patients daily.

Qualifications

• State RN license required.
• 1 year of experience in in-home care.
• Excellent communication and time management skills.
• Compassionate and friendly demeanor.
• Ability to travel to multiple patients per day.
• Preferred knowledge of WellSky Kinnser.

Responsibilities

• Assess patients' condition during every visit and chart observations.
• Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy.
• Dress or re-dress open wounds and assess healing progress.
• Educate patients and their families on home health care strategies.
• Coordinate with therapists and physicians involved in patient care plan.
• Make recommendations for devices or tools to improve quality of life.
• Listen to patient concerns and respond to requests.

We work with patients of all ages and backgrounds who require Intermittent Skilled Care in their home. We hire the most compassionate and thoughtful talent in the field. Home Health Nurse Responsibilities

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy
• Dress or re-dress open wounds and assess the progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Speech Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests
