Residential Home Health and Residential Hospice in Lansing, MI is seeking a Home Health RN to deliver skilled nursing care to patients in their homes. Qualifications

• Active RN license in Michigan.
• At least 1 year RN experience delivering home health care.
• Valid driver’s license and ability to drive to patient locations 20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and ensure thorough EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
