Job summary

Residential Home Health and Residential Hospice is seeking a Home Health RN in Bad Axe, MI to deliver skilled nursing care in patients' homes. You will assess, plan, and coordinate care while traveling within the assigned territory.

A $10,000 first-year bonus is offered along with comprehensive benefits and opportunities for growth within a nationally recognized home health and hospice leader.

Qualifications

• Active RN license in the state of work.
• One year RN experience with clinical expertise to deliver quality patient care.
• Current driver’s license and ability to travel ~20% of the day to patient locations.

Responsibilities

• Delivers skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications
