Job summary

Residential Home Health and Hospice in Michigan is seeking a Registered Nurse (RN) for a Home Health position to provide skilled nursing care to patients in their homes in Dexter, Chelsea, Ann Arbor, and surrounding areas. This role offers a $10,000 first-year success bonus to reward achievement during the initial year.

The RN will perform patient assessments, medication management, and care planning while educating patients and families on disease management.

Qualifications

• Active RN license in the state you work.
• Minimum of 1 year RN experience delivering patient care.
• Must have a valid driver’s license and be willing to travel up to 20% of work time.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and ensure compliant EMR documentation.
• Work independently, but collaborate with the team when needed.
• Travel to patient homes within the assigned territory.

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
