Job summary

Residential Home Health and Hospice seeks an RN to provide skilled nursing care in patients’ homes across Howell, MI and surrounding areas. The role includes travel within the territory and eligibility for a $10,000 first-year bonus for achieving key milestones.

You will collaborate with an interdisciplinary team, perform assessments, manage medications, and educate families to empower self-care and adherence to plans.

Qualifications

• Active RN license in Michigan.
• Minimum of 1 year RN experience delivering skilled nursing care.
• Valid driver’s license and ability to drive ~20% of the day.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
