Job summary

Residential Home Health and Residential Hospice seeks a dedicated Home Health RN to provide in-home skilled nursing care across Novi, Wixom, Commerce, and surrounding areas. You’ll lead patient assessments, medication management, and care planning, empowering families with knowledge and confidence.

You will work independently within a supportive interdisciplinary team, traveling to patient homes and documenting care in the EMR.

Qualifications

• Active RN license in the state you work.
• One year of RN experience in home health or relevant clinical setting.
• Valid driver’s license and ability to drive to patient locations ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in patients' homes, collaborating with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate families on disease management and treatment plans.
• Document thoroughly in the EMR and maintain compliance.
• Travel to patient homes within the assigned territory.

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
