Graham Healthcare Group is hiring an Registered Nurse for a Home Health role covering Novi, Wixom, Wolverine Lake, Walled Lake, Commerce, and surrounding areas in Michigan. You will travel to patient homes to deliver skilled nursing care and work with an interdisciplinary team to ensure quality outcomes.

The position emphasizes autonomy, patient education, and comprehensive care planning, with emphasis on accurate EMR documentation and patient-centered care in a home setting.

Formación

• Active RN license in the state you work
• Minimum of 1 year RN experience delivering patient care

• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
