Elara Caring is seeking a Home Health RN Case Manager for the Northampton-Easthampton territory. The role is full-time, offering a guaranteed salary of $87,000 to $97,000 plus a productivity bonus, mileage reimbursement, and flexible day planning.

You will lead skilled nursing visits, care plans, and patient education in patients’ homes. You’ll collaborate with an interdisciplinary team, manage an assigned caseload, and coordinate with physicians.

Qualifications

• Massachusetts RN license (current and unrestricted).
• At least 1 year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to travel throughout the assigned territory with valid driver’s license and reliable transportation.
• Ability to lift 50–100 pounds as needed.

Responsibilities

• Provide skilled nursing visits and discharge planning.
• Develop and update care plans for the assigned caseload.
• Coordinate with physicians and other clinicians.

What You’ll Do

• You’ll guide patients through their home health care, from assessment to discharge.
• That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians.
• You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

What You’ll Bring

• Current, unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment, communication, and time management skills
• Ability to work independently and travel throughout assigned territory
• Valid driver’s license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50-100 pounds
