Elara Caring is seeking a Home Health RN Case Manager to deliver high-quality in-home care across Attleboro, Foxboro, Walpole, Sharon and Easton. You will perform skilled nursing visits, develop and update care plans, manage an assigned caseload and coordinate with physicians and clinicians to ensure patient recovery and independence.

What You'll Do

You'll guide patients through their home health care, from assessment to discharge. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians. You'll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

What You'll Bring

• Current, unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment, communication, and time management skills
• Ability to work independently and travel throughout the assigned territory
• Valid driver's license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50-100 pounds
