Job summary

Residential Home Health and Hospice in Bad Axe, MI is seeking a Registered Nurse to provide skilled nursing care in patients' homes. Eligible for a $10,000 first-year success bonus and ongoing bonus opportunities as you grow with the company.

You will conduct patient assessments, manage medications, and develop care plans, educating families on disease management. Expect travel within the territory and autonomy within an interprofessional team.

Qualifications

• Active RN license in the state you work.
• At least 1 year of RN experience delivering home health care.
• Valid driver's license and ability to travel ~20% of your day.

Responsibilities

• Delivers skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that supports you.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

RN Qualifications
