Graham Healthcare Group seeks an experienced Registered Nurse (RN) for a Home Health position serving Harbor Beach, MI and surrounding communities. You will deliver skilled, patient-centered care in patients' homes, collaborate with an interdisciplinary team, and travel within the territory.

Eligible for a $10,000 first-year success bonus; we offer comprehensive benefits, paid time off, retirement plan, and opportunities for professional growth.

Qualificações

• Active RN license in state you work.
• 1 year of RN experience delivering quality patient care.
• Valid driver’s license and ability to travel to patient locations 20% of time.

Responsabilidades

• Delivers skilled, patient-centered care in home health with interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• One year of RN experience andthe clinicalexpertiseto deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
