Job summary

Trinity Health At Home – Grand Rapids (Michigan) is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes, developing care plans, administering medications, and educating families to support recovery and independence.

This full-time role offers meaningful 1:1 patient relationships, no on-call requirements, comprehensive benefits starting day one, and opportunities for growth within a faith-based, not-for-profit home health system.

Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Minimum of one year professional registered nursing experience.
• Medical-Surgical experience preferred.
• Home health or Hospice experience is a plus.
• Strong computer and technology skills.
• Must have current Driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and monitor patient progress.
• Educate patients and families to support recovery and independence.
• Collaborate with the care team to coordinate services and care plans.
• Support 1:1 patient relationships and empower patient autonomy.

Minimum Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
