Job summary

Trinity Health At Home - Muskegon is seeking a Home Care RN to provide primary nursing care at patients' homes, develop individualized care plans and collaborate with the Population Health Case Manager to ensure quality outcomes.

You will conduct assessments, implement plans, educate patients, monitor changes, document care, and mentor team members. Day One benefits apply, and a sign-on bonus of up to $15,000 is offered.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current RN license in the state of practice.
• Minimum one year of professional nursing experience; home health experience preferred.
• Strong computer and technology skills.
• Current driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments at admission and ongoing intervals.
• Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.

Position Purpose
