Job summary

Trinity Health At Home – Grand Haven seeks a Hospice Registered Nurse Case Manager (RN) to deliver skilled nursing care in patients' homes, develop individualized care plans, administer medications, and educate patients and families. You will be part of a nurse-led team delivering compassionate, patient-centered care across the Grand Haven, Grand Rapids or Muskegon area in Michigan.

Minimum qualifications include an active RN license in Michigan and at least one year of clinical nursing

Qualifications

• Active Michigan RN license
• 1+ years of clinical nursing experience (Hospice preferred)
• Strong communication, assessment, and organizational skills
• Compassionate, dependable, and mission-minded
• Valid driver’s license and reliable transportation required

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans
• Administer medications and educate patients and families
• Build real relationships and provide meaningful 1:1 patient care
• Coordinate care with the broader Trinity Health At Home team
