Job summary

Trinity Health At Home - Grand Rapids is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes across Wyoming, Alto, Caledonia and Kentwood. You will develop individualized care plans, administer medications, and educate patients and families to support recovery and independence.

Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Minimum of one year professional RN experience.
• Medical Surgical Experience preferred.
• Home health or Hospice experience is a plus.
• Strong computer and technology skills required.
• Must have current Driver's license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and educate patients and families to support recovery and independence.
• Provide 1:1 patient care with strong clinical judgment and compassionate communication.
• Collaborate with care teams and coordinate services to ensure seamless care.
• Travel to service areas Wyoming, Alto, Caledonia, and Kentwood as needed.
• No on-call requirements and flexible scheduling.

Minimum Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
