Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice Registered Nurse Case Manager to deliver skilled nursing care in patients' homes, develop individualized care plans, administer medications, and educate patients and families.

Qualifications

• Active RN license in Michigan.
• 1+ years of clinical nursing experience (Hospice preferred).

• Valid driver’s license and reliable transportation required.
