Job summary

Trinity Health At Home – Muskegon is seeking a Homecare RN to deliver primary nursing care in patients’ homes and to develop individualized care plans in collaboration with the Population Health Case Manager.

You will conduct comprehensive assessments, implement care plans, provide therapeutic interventions, and educate patients to promote independence and better health outcomes. Documentation and mentoring of the team ensure quality care delivery.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current RN licensure in the state of practice.
• Minimum one year of professional nursing experience; home health experience preferred.
• Strong computer and technology skills.
• Current driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments at admission and ongoing intervals.
• Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.
