Qualifications

Location: Around Jefferson Park, IL – Edgewater, Elmwood Park, Park Ridge, and surrounding areas.

Qualifications

• Graduate of an accredited Nursing Program with an active RN license in IL


• Minimum 2 years of nursing experience, ideally in home healthcare


• Current CPR and BLS certifications


• Knowledge of state and federal home health regulations


• Strong clinical judgment, excellent communication skills, and a compassionate approach


• Bilingual Spanish, Polish, Russian, Korean, etc. is a wonderful plus; but not required

Compensation and Benefits

  • Full‑Time Employees: $85,000 – $105,000 (+ mileage)
  
  
  
  
  • Five‑day schedule: Monday – Friday or Tuesday – Saturday
  
  
  
  
  • PRN/Per Diem Employees: Earn between $60 and $165 (+ mileage)
  
  
  • Baylor Program Employees (every other Saturday and Sunday) earn $50.00 bonus for SOC and ROC OASIS visits

Your Role and Responsibilities

• Provide home‑based care to patients recovering from illnesses, injuries, and chronic conditions


• Collaborate with a multidisciplinary team to ensure delivery of high‑quality care


• Examine patients, conduct assessments, document progress, and effectively communicate with other healthcare providers


• Educate patients and their families on care plans, medications, therapies, diet, and exercise


• Coach families on care strategies and home safety


• Ensure care goals are met and coordinate discharge from services
