Job summary

Interim HealthCare of Northern Illinois is seeking a Home Health Registered Nurse (RN) to deliver personalized nursing care in patients' homes across Rockford and surrounding areas. You will work with a team and oversee LPNs/CNAs, perform assessments, and educate patients on care plans, medications, and safety.

We offer top pay with a base salary in the $80k–$95k range, potential bonuses each pay period, mileage reimbursement, on-call pay, and a 1:1 nurse-to-patient ratio to positively impact

Qualifications

• Graduate of an accredited nursing program and active IL RN license.
• Minimum 1 year of nursing experience, preferably in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Strong communication and compassionate patient care.
• Clinical proficiency in nursing specialty.

Responsibilities

• Provide home-based care to patients in their homes.
• Oversee LPNs, CNAs and aides providing patient care.
• Examine patients, perform assessments, document progress.
• Educate patients on plan of care, medications, diet, and exercise.
• Coach family on patient’s plan of care, home safety.
• Coordinate discharge from services.
