Job summary

Interim HealthCare Inc. is seeking a Home Health Registered Nurse in Illinois. The role focuses on assessing home care needs, coordinating services, and educating patients and families on care plans. The RN will document progress, communicate with other providers, and support transition planning for patients returning home.

Qualified candidates hold an IL RN license, CPR/BLS, and 2+ years in home health or similar settings. A comprehensive benefits package and mileage reimbursement are offered.

Qualifications

• Graduate of an accredited Nursing Program with an active IL RN license.
• Minimum 2 years of nursing experience, ideally in home healthcare.
• Current CPR and BLS certifications.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, excellent communication skills, and a compassionate approach.

Responsibilities

• Assess the patient’s and family caregiver’s homecare needs and coordinate home health care services as required prior to the patient’s transition home.
• Act as a clinical resource to coordinate complex cases for safe and appropriate transition to the patient’s home.
• Examine patients, conduct assessments, and document patient progress while effectively communicating with other healthcare providers.
• Educate patients and their families on care plans, medications, therapies, diet, and exercise.
• Complete and present patient reports to facilities and management teams.
• Work collaboratively with the Office Intake Department in verification of coverage or payment.

Compensation and Benefits

• Competitive Compensation: Full-Time Employees* — $90,000 - $110,000 (+ mileage)
