Infinity Hospice Care LLC is seeking an experienced RN to administer nursing care in clients’ homes across Texas. You will coordinate with interdisciplinary teams, patients, and families to deliver comprehensive home health services.

The role requires an RN license with one year of clinical experience; home health experience is preferred. Strong communication, reliability, and English proficiency are essential for success in this role.

Qualifications

One year experience as an RN in a clinical care setting.
Home health experience preferred.
Must read, write and comprehend English.

Responsabilités

Promote /exemplify the Company's mission, vision, and values.
Complete initial and ongoing assessments to identify client needs, including coordinating with the interdisciplinary team.
Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
Provide effective communication to clients, families, and health professionals; participate in case conferences.
Coordinate services and inform physicians of changes in client condition and needs.
Monitor cases for compliance with third-party payers; ensure timely documentation.
